How can she be handed over when she has a sentence to serve?

Yes, but even that has a due process which must be followed. Pakistan has to initiate the process for its citizen first.

I don't know how true the following is but it is said that she is a scientist, brilliant biology scientist specializing with something genetics and had discovered something unique that could assist in creating super soldiers. There are also rumors that she had made some discoveries that could easily be weaponized to create extremely powerful biological weapons that would dwarf the amount of destruction nuclear weapons would create. But being a woman of strong faith, she destroyed all of her research, the only place it exists is her mind and the u.s. wanted her to divulge that info to them but she won't so they put her in prison until she did with a sham case.

USA can initiate the process unilaterally even though I do not see why they would release a known "terrorist"

Aafia is not going to be released, not under Trump. Pakistan will have to offer something in exchange, be it Shakil Afridi or something else.

Stranger things have happened in international geopolitics. If the national interests of both sides coincide sufficiently over her, then so be it.
