Probably patient did not feel much but violent muscle contractions were indication of inadequate anesthesia. Some times it is human error and sometimes it is old beaten up anesthesia machines.are you a surgeon or the patientit is really sad we do not even bother to follow proper guidelines
mostly human error in my opinion but yes sad to see that we can not even document and learn from errors because of inflated egos

No, I work in a multinational engineering firm where we teach newly employed staff on how to use the basics of design tools.
