It was tragic accident in bahria university Islamabad where daughter of officer died by falling from university building under construction due to lack of fences and commencement of classes in campus before completion of construction of building

Basically the victim leaned against a soft screen barrier and fell to her death down the shaft.

Where a locked door / hard barrier should have been to stop access to the wide shaft for pipes running down the building, there was a soft coloured screen only.
