So the buck should stop with the head of the agency and the minister concerned.
ISI should be renamed to reflect external/foreign intelligence role and not army role and report to foreign minister and prime minister not army chief. It should have career agents and not army majors on rotation. Army staff are no more capable than any skilled civilians. Army staff only needed for foreign special operations, but under control of civilians.
