It is an unguided test. The standars PL5-5e (see serial on fins) without the steering frontal fins.

I am pretty sure that these images were taken from the modified fuel tank that PT-04 use to carry in different tests, yes it carried cameras in it. This fuel tank has two holes / windows at front and rare section, as you can see.
Sorry but I couldn't find any clear picture of it. Maybe Najam had it on his website but its not there now.
