Realistically, I see a combination of factors converging:

1. India announces some major political change to Kashmir
2. India covertly begins Israeli style settlements to change demography
3. Pakistan issues a major warning to cease immediately
4. India continues and makes botched attempt to annex some of Pakistani Kashmir
5. Major armed confrontation takes place between Pakistan and India
6. Both deescalate, if one side rejects, both go to war.

Other factors could really serve as a catalyst to ignite an Indo-Pak conflict into an open confrontation.

Biggest would be if Trump gets elected and Iran decides its ready to go to war with US.

Pakistan gets distracted trying to do damage control between Iran and US and then India smells blood in the water and decides to launch an advance in Kashmir.

I do not think India will use above as factors for an all out war. Hindutva is a reactionary ideology one that feeds on hate and Indian politicians use it wisely to distract people from their daily grind especially when it gets heated. So, the assumption is if domestic compulsions push India to heat up LOC and one things leads to another going up in escalatory ladder or another Pulwama type incident starting a frenzy for a bigger and more direct response are more likely scenarios. Remember Kashmiris are still in lockdown already situation where much of Kashmiri population has been alienated in a single swipe by hinduvadis only for brownie points knowing fully well the repercussions will mean losing trust of Kashmiris for ever! So, I don't think there is any turning back for Kashmiris at this point or in foreseeable future. Current paradigm cannot last in perpetuity.
