Not in every case but certainly true in the Indian context..
In the worst hit disasters... When people are dying of imminent death... Then too it comes with a lot of strings attached. That's how government's work.
The only one that is not is where the giver doesn't feel the need to advertize their contribution, which may be the case when done by individuals, but not nation states.
There is a tradition that narrates Prophet Muhammad saying that:
"There are seven whom Allah will shade in His Shade on the Day when there is no shade except His Shade: a just ruler; a youth who grew up in the worship of Allah, the Mighty and Majestic; a man whose heart is attached to the mosques; two men who love each other for Allah's sake, meeting for that and parting upon that; a man who is called by a woman of beauty and position (for illegal intercourse), but be says: 'I fear Allah',
a man who gives in charity and hides it, such that his left hand does not know what his right hand gives in charity; and a man who remembered Allah in private and so his eyes shed tears.'" (Abu Hurairah & collected in Saheeh al-Bukhari (English trans.) vol.1, p.356, no.629 & Saheeh Muslim (English trans.) vol.2, p.493, no.2248)
There are two reasons why people make a show of all their giving to the poor
1) To extract benefit from the receiver.
2) To incease their own social standing among other givers.
Nation states' aid usually falls in the first category and most other people's in the 2nd.
