It's not the capital city. He first arrived in Mashhad city for a brief visit. The official ceremony will be held tomorrow in Tehran.

its bigger and bold step by Pakistan , there could be a big lash back by west on Pakistan.

Iran should not politicize this step and roar towards west . But look for opportunity to improve trades and relations , same goes for Pakistan.

Also Iran is not friendly towards Pakistan , it considers Pakistan as KSA puppet. As iran has still dumb govt, who wants things black or white, they dont understand diplomacy being done all around the world where everyone should be friend , even your enemy. Iran visa policy for Pakistan is big answer here

Dont expect big trade agreements due to sanctions by US

Image of Pakistan in Iran is that of a third rate citizen state, where Indians can go without visa but Pakistanis are only allowed to Mashahd once their visa is stamped in Islamabad.
Again, Pakistanis are only allowed to immigrate via Mashhad, only in special cases other port of entries are allowed.

Pak need to keep a neutral stance like in Yemen and not be proxied by KSA or Iran in their hate for dominance.
