Does it state PAKISTAN requested these, or where they offered to PN by the US.

As per this DATA it was requested by Pakistan (as per EDA rules & procedures the acquiring country has to forward a formal request ) but it very difficult for me to comment that these helicopters were actually requested by Pakistan

If requested by PN, what was the reason for US delcine (was there US decline, or was the offer to expensive?).

Now here is the twist on the basis of which we can assume that those Helis were actually on offer to PN as the column 'EDA Rejected' is blank so USA has not rejected or declined that ''request'' of PN (not even the quantity were negotiated which generally happens with requests in advance stages) on the other hand the column 'EDA Cancelled' showing Jan-06-2012 mean Pakistan Navy taken her request for those helis back on that date.

BTW offer was not expensive at all as the Unit price was just $4.6 million or $28 million for all 6 helicopters on offer/ (requested)

It would not be the first time a state is offered equipment and doesn't pick it up e.g. Spruance class destroyer was offered to PN but PN declined.

I believe this is the case with this request as well.

So, what was the offer? And will PN going to give its Sea King to private people to use it or convert it to spares?
