This kind of threads should be started more often,they show sanity still prevails among us.
On a serious note,I don't think this particular problem is unique to PA,every other armies may suffer from same problem.In times of battle this may lead the lives of soldiers to potential danzer.In my opinion,soldiers should shave off their heads like those US Marines.

Yeah, it's difficult to scratch especially when the itch is just on the top of your head (inching on sides can quite be managed, no?), solution; have a hole at the hop!

The helmet has both entry and exit openings, with suggestion of a large caliber projectile that passed through largely intact. Survival would be unlikely without timely surgery, as you point out.
