Frankly, no one can predict the outcome of a nuclear war on South Asia.
Neither Pakistan nor India currently has a nuclear triad, period. Hence, the idea of MAD is a non-starter so long as the lacuna exists.
Given this fact, the actual number of nuclear weapons that the two Nations are effectively able to exchange in a War is extremely speculative. Given that the country that initiates the exchange does so on a massive scale, chances are that the nuclear chain of command of the adversary can be hit to an extent that a retaliatory strike might not even take place.

wel since india has no first use policy, it would be pakistan that would have to hit hard enough that we get no response from india.

Possible. Which is why India is so desperate at setting up a fleet of nuclear subs. Only with a fleet of such subs which can collectively carry enough nukes to assure MAD can India be truly sure that Pakistan will not ring in the first nuke shots on the subcontinent.
