could have passed out amid surgery and messed it up...

Taking risks is crucial for doing something amazing. I am not a surgeon, but he is, so I am sure he would have analyzed the risks before performing the surgery on himself. Besides, the photo caption says that he "was the only surgeon in the Soviet Antarctic Expedition". This means that he may have had no other choice, but to operate on himself.
