This only happens when there is an investigation or a "tip" provided.
In a normal procedure, immigration has nothing to do where you go and whether you have visa or not.
When you check in, its airline, (whose *** is involved) which makes sure you got visa or not for your destination.

Airlines face fines because they have to freakin operate there and hence a norm.. and home country immigration is not easy to be contacted due to red tape. The main responsibility is of your home country's immigration to ensure that a wrong person on forged documents is not permitted to leave. The problem is that you have considered it a norm or 'way of life' that immigration should only move their arse when a so called tip is provided.
