Does it make a difference if we would have been attacked first? Pakistan although conducted its test in 98 is believe to have carried out its cold tests since the 80s.

I dont think you can decide what a usual target would be once you have hit another country nuclear reactor. Any country and not just with nukes would count that as a nuclear attack and respond accordingly.
