I think a problem with the war in Vietnam was incidents like this happening because the men patrolling the bush were conscripts rotated out on a regular basis. Just when the US troops were figuring out how to fight the VC/NVA, they would be sent home and fresh-faced oakes sent to replace them. Accordingly, the tactical competence of the men suffered - resulting in serious losses.
Add that to the fact that Washington treated the Vietnam War as a political war rather than a military one, and their COIN effort was doomed.
