Has anyone applied for a US visa from Canada? Is it possible to get it here or does the application have to be made back in Islamabad?

If you are a resident of Canada then you can, i.e. you're not there on a visit.

But this is more anecdotal. I've applied for visit visas to US only from Dubai. I'm a Pakistani national never had to go to Islamabad to apply for any visa since I'm a resident of Dubai.

Okay, I found the answer to my own question, thank you for helping.

If you have a prior expired US visa, then you can apply in Toronto for a new one. First time applications for US visa must be made in the home country.
