This old news says that the plane was not cleared for flight by engineers but was flown on the instructions of PIA flights director.


This report says that a turbine blade had to be changed at 10,000 hours and wasn't. The blade Broke inside the engine during flight and brought down the plane.
The blade had gone only 4.1 hours above recommended part change time.

And what happened? Was there an investigation? Did the responsible person loose his job? Were there any changes made to the “system”?
Looking at the recent crash, I am guessing no.

They would've been quietly be transferred to another department. No one gets fired.
