You do realize that there is a significant number of PDFers that actually live there. There are thousands of videos on youtube of Gilgit Baltistan and Azad Kashmir. Millions of Pakistanis visit there every year as a part of domestic tourism...

Most Indian citizens probably might have never heard of BLA.

The very first episode of Saif Ali Khan's Sacred Games TV show shows a RAW agent watching a video message from Baloch insurgents. So maybe ordinary Indians haven't heard of BLA but they must know that India is supporting Baloch insurgents in Pakistan. It must be common knowledge by now.

BTW I only watched that one episode but one thing I found funny about this show was the huge contrast between imported technology and the indian government system. Like he drives a nice car and has a smartphone but when he needs to look up old cases he has to go digging through paper files.

Most Indian citizens probably might have never heard of BLA. Even I came to know about these groups in Pakistan after I visit PDF some 6 years back (without joining). Indian politicians who comment on them are having a knee jerk reaction to Pakistan sponsored separatist movements in JK.

What I am saying is, we couldn't care any less about what you do with your own people. There are no large association with Pakistani groups with Indians. They hardly make any news unless there is a major attack like APS.
So, you carpet bombing them all is your prerogative, if any unknown BLA commander acquire passport of Afghanistan and come to India for treatment, then there isn't much to it other than they are not in travel ban list in airport and their visa is valid given India accept medical tourists from Afghanistan fairly quickly compared to Pakistan. Even with this US banning, they could still move freely to India for medical treatment.
