There are differences between misinformation and disinformation. When any government uses it's own sources to distort truth, that is misinformation. When the government uses private sources such as foreign news portals to either distort or fabricate truth, that becomes disinformation. As people often mistrust government sources, it is the disinformation which becomes more effective way to distort/fabricate truth. These are all tools of Psychological warfare at the end of the day.
In a nutshell, an ordinary reader isn't simply capable of distinguish facts from sophsticated disinformation/misinformation campaign unless that reader r has good analytical mind with intensive training in psychological warfare.
