from what we know yes he is terminally ill
if despite that he is coming then there are few possibilities
one being, like you pointed out that its for staying relevant or publicity (and in case he is not ill at all)
second being, its an emotional decision and since he is terminally ill he wants to die in Pakistan and at this stage doesn't really care about court cases and criticism.
finally I agree with you that he will do whatever to stay relevant
Musharraf's political career is pretty much over and not more that late Asghar Khan or Sahabzada Nasar Ullah Khan.
. this is the thing with power of becoming the leader. whoever got it once always came back for it. despite all the dangers.
Sherifs will keep trying to stay relevant no matter how much their opponents want them to go away. Benazir came despite all the threats and died among with hundreds of ordinary Pakistanis after repeated back to back attacks from the time of her landing in Karachi to her assassination in Pindi. Bhutto had the chance to go in exile but he stayed on (believing the courts wont do the unthinkable but they did). Gen Zia had an indication that his time was up and he couldn't give up till his crash.
I think its an emotional decisionbakwaas he should sit there otherwise he will be dragged in courts until he is alive . just forget paksitan sari zameen allah ki hai
he just wants to go down fighting. he will indeed be humiliated in courts and all but it seems he doesn't care now as he is terminally ill

He is not a traitor after All, he has right to live and die in Pakistan's soil. So that other patriots will have courage to come back in Pakistan especially in the time of war.
So kindly make an example for the patriots. Let him face all the Cases in Courts, but I am in favour of his returning to Pakistan.

External Funding , and Resources were used to oust Musharaf from power
a) Fanatics were involved (Lal Masjid)
b) TV channels helped spread lies and false news to ignite anger in people
c) Lawyers drive helped push the country towards chaos
d) Then came external pressure on Pakistan
National Reconciliation Ordinance , we should use the full words so younger audience understand what exactly is NRO , it was promised or Idea was sold to Musharaf , that once the country will be in hand of experienced
Politicians like Nawaz Sharif, and Zardari they can control people at ground level to change the course of country. They together with their connections in their clans and circle of influence can help keep the population controlled
Under National Reconciliation Ordinance, all the past corruption cases from which these families ran away with their wealth , were forgiven , and they were allowed to bring back money back to Pakistan to start the next cycle of National Rape on Pakistani economy
Between 2007 and 2018 Country saw a period of 10 years of Darkness
No country in the world has such a Law where a group of people who are being investigated for Serious crimes and corruption are forgiven and not only forgiven but allowed you be in power
What ever forces lead Musharaf to accept National Reconciliation Ordinance as a means to move country forward are Traitors to Nation of Pakistan
The special thing about these criminal elements is when they need to get something done they will become extremely polite and cooperative and they will act as they are your best of friends and they care about Pakistan , but once their key agendas are met they will go back to their old ways and that is what happened to Musharraf. Once these Corrupt politicians were allowed back into country , they started to spread their poison , and cripple the nation and Musharraf was forced out
If 10 years were spent under Musharaf's rule , Pakistan would be in a very very stable economic condition , Pakistani Rupee traded at 1 dollar for 55 rupee value under his time
