It was a parachute malfunction. Fourth incident of this kind in the past 10 years. RIP Commando.

I hope as review will be done of the people and process involved in re-packing the parachutes.Parachutes are designed such that the probability of both main and reserve failing is pretty low. And in modern ones the secondary is launched through a separate mechanism so human error is minimized. As far as I know, the main problem when using the secondary is line entanglement

Oh OK. I have done a full sky diving course in Australia and as part of that we were taken to the area where dedicated people repack them. I guess procedures can vary between countries and civil vs military.
