A sudden turn made by a US destroyer lead to a collision with a commercial vessel that killed 10 American sailors, an investigation by Singaporean authorities has concluded.
The incident on August 21 was one of several such collisions last year, which tarnished the reputation of the US Navy. The guided-missile destroyer USS ‘John S McCain’ (DDG-56) collided with a Liberian-flagged merchant ship in the Singapore Strait.

A report released on Thursday by the Singapore’s Ministry of Transport's (MOT) Transport Safety Investigation Bureau (TSIB) said the American warship “made a sudden turn to the left into the path of [the merchant vessel, ‘Alnic MC’] because of a series of missteps that took place after propulsion controls were transferred.”

The US Navy’s internal report last November arrived at similar conclusions, blaming the incident on avoidable navigational errors made by the destroyer’s crew.

The incident that occurred near Singapore last August happened just two months after a similar collision involving the destroyer’s sister ship, USS ‘Fitzgerald,’ near the Japanese capital Tokyo. On June 17, she collided with a container carrier, resulting in the deaths of seven sailors. This case was also blamed on lapses of the warship’s bridge team.

Five US sailors have been reported injured and 10 are missing after the USS John McCain collided with a merchant ship early August 21 near Singapore and the Strait of Malacca.

The US guided-missile destroyer crashed into the Alnic MC tanker while underway east of Singapore, the US Navy 7th Fleet reports. The extent of the five sailors' injuries is not yet known.

The incident occurred at 5:24 a.m. local time as the ship was on its way to make a routine port visit in Singapore. The McCain has been damaged on its port side aft, or left rear, the Navy says. The latest US Navy update has the ship sailing under her own power toward port. The status of the tanker is unknown.

A search and rescue operation has been launched, combining the efforts of the Singapore Navy and US ships in the area. The Singapore Navy ship RSS Gallant, Singapore helicopters and the Police Coast Guard vessel Basking Shark are currently in the area to render assistance, the 7th fleet reports, and Osprey aircraft and Seahawk SH-60 helicopters from the amphibious assault ship USS America are also joining the response.

The incident will be investigated, the Navy has said. The Twitter feeds of the US Navy and the Pacific Fleet are displaying a phone number for the families of crew members to call for information.

The USS John McCain is an Arleigh-Burke class destroyer belonging to the US Navy's 7th Fleet, and is home ported at the US Navy base in Yokosuka, Japan. The Alnic MC is described as a Liberian-flagged, 30,000 ton, 183-meter oil and chemical tanker by the Marine Traffic website. It appears to have been on its way to Singapore from Pyeongtaek port in South Korea.

"Our first priority is determining the safety of the ship and crew," Admiral John Richardson, chief of naval operations for the US Joint Chiefs of Staff, said in a Twitter statement.

This accident comes only weeks after the June 17 collision of the USS Fitzgerald with the ACX Crystal container ship, in which seven of the Fitzgerald crew were killed and several others injured. A high level staff shakeup followed the crash.

The Straits of Malacca and Singapore are the world's busiest shipping lane and reached a new high of 83,740 transits in 2016, according to Seatrade Maritime News.

The collision between John S. McCain and Alnic MC was also avoidable and resulted primarily from complacency, over-confidence and lack of procedural compliance. A major contributing factor to the collision was sub-standard level of knowledge regarding the operation of the ship control console. In particular, McCain's commanding officer disregarded recommendations from his executive officer, navigator and senior watch officer to set sea and anchor watch teams in a timely fashion to ensure the safe and effective operation of the ship. With regard to procedures, no one on the Bridge watch team, to include the commanding officer and executive officer, were properly trained on how to correctly operate the ship control console during a steering casualty.

Now back to the story, i believe that moron jhungary was writing a whole essay pointing the mistake was made by the commercial vessel. Tada why aren't we surprised, this so called military professional was wrong from the very beginning and it didn't take a genius to figure that out. When there are 4 incidents involving the US Navy in the Asia Pacific crashing with commercial ships you know who the culprit is. But gambit and jhungary were not convinced :D
Now we really have to doubt the worthy of the professional title to these so called military experts.

Dear Chinese, He made fun of our dead soldiers and incident. That is what I objected to and I did not try to absolve us of our human error. Thank you for your post as yet another case study about the famed IQ you guys brag about. The land where accidents and 30 million starved to death is covered up as western lies.
