Is it legal between direct cousins? That is, between the offspring of say, a brother and a sister?
yes it's completely legal.even in some tribal area it is encouraged .
 
Despite the known genetic problems it can cause? Is it really religiously sanctioned?

Yes, it's allowed since Muhammad (SAW) did it. Clerics should definitely speak up about it since times have changed and now we understand that cousin-cousin marriages can cause problems. No one really knew much about genetics a thousand years ago.

I do agree with you. But I guess the question is are people at large willing to recognize that what the Prophet Muhammad did - at least in this instance - was perhaps not the thing to do? (I didn't know the Prophet Muhammad married his cousin). As in, a lot of Muslims think the Prophet Muhammad should be emulated as he was the perfect Muslim.

Muhammad (SAW) wasn't a firm, strict individual like many people say regarding everything. The fundamentals of faith were unquestionable (belief in God, no polytheism, etc) but when it came to other matters, He was flexible and pragmatic, which is not what many people understand.
