Because the mullah is scared. Simple as that.

Khamenei is smart enough -- or at least his advisers are -- to understand that for the West, charismatic figures are transitory, meaning Trump, Obama, Bush, Raygun, et al, comes and goes. He knows that proportionally, none of them wields internal power and authority the way he, and other dictators, wields power over their respective countries. Trump maybe labeled 'The Leader Of The Free World' but internally, he is weakened by the Congress, the Supreme Court, and the 50 states by way of a piece of paper called the Constitution.

But 'Death To America' is not 'Death To Trump/Obama/Bush/Clinton/Bush/Reagan/etc...etc...etc...'

It is calling for the erasure of an entire people, culture, and even place marker in history. At the extreme, it is calling for the US to never exist in the chain of history completely.

The call 'Death To America' can only be made by another state. Never by an individual. Khamenei as an individual is essentially insignificant. But Khamenei -- or whoever before and after -- as the highest agent of Iran, it is effectively the State of Iran calling for the utter destruction of another state. Every Jew knows the philosophical importance of 'Death To Israel' and every Israeli takes it seriously. Sadly for Americans, most of us do not have the same attitude for 'Death To America'. But on a positive note, if Americans do take 'Death To America' to the same seriousness as the Israelis take 'Death To Israel', there would be weekly gatherings of Americans in the streets returning the favor to any country that calls for our destruction.

But Khamenei is scared of Trump. B43 was unpredictable and not only Afghanistan, Iraq fell under his administration. Two countries in near proximity to Iran and there was nothing Iran could do about it. The Orange One frightened American allies. Never mind the Euros' disgust at him, most of Americans who voted for him were/are disgusted as well. But Trump is a frightful figure on the world state. A unsophisticated, boorish, and buffoonish clown with a very large knife that you can only laugh at him behind his back or behind closed doors. Trump's personal narcissism extends to the US at large. He views the US as personally attached to his ego and that made him unpredictable, hence, dangerous to competitors, adversaries, and outright enemies.
