Pakistan has been at a constant state of war for the last decade so that excuse doesn't hold even for comparison purpose. Machines and people get overworked, it's a management and planning issue to control.
People's lives are very precious, and this is an unfortunate event. If technical reasons than the maintenance causes should be found.
